An On-site Access Plan keeps our medical team on call for your jobsites. Every visit, test, and follow-up is billed from one published fee schedule, so there are no surprises on the invoice.

The annual fee covers availability and response during service hours. Services rendered are billed separately at the fee schedule in effect on the date of service.
On-site Access Plan
Billed once per calendar year. Keeps the triage line and mobile team available to your jobsites across the Denver metro.

How billing works
Two line items, one monthly invoice
Annual access fee
$600 per calendar year, non-refundable. It pays for on-call availability and response during service hours.
Services as used
Each visit, test, procedure, and follow-up is billed at the fee schedule rate in effect on the date of service. Requests outside Monday–Friday, 7 a.m.–4 p.m. add the After Hours Fee.
Monthly invoice
Services are invoiced monthly, due net 30. Past-due balances accrue 12% annual interest.
No agreement? Service Fee
Employers without an active agreement can still request care; a $75 Service Fee is added to each incident.
The requesting employer is the financially responsible party. ICA does not bill on-site services to insurance carriers or third parties; any insurance billing is the employer’s responsibility.

Hours, service area, and response
Coverage runs 6 a.m.–11 p.m., seven days a week, on every plan. Outside the standard service area, service is available based on team availability, with the Extended Travel fee added.

Weekends and six national holidays (New Year’s Day, Memorial Day, Independence Day, Labor Day, Thanksgiving and Christmas) carry the After Hours Fee. *The 2-hour response is a best-efforts target, not a guarantee; requests outside regular hours may take longer. 11 p.m.–6 a.m. is not covered. In an emergency, call 911 immediately.

What you’re agreeing to

Term and renewal
One-year initial term starting the first of the month after signing, renewing automatically for successive one-year terms.
Ending the agreement
Either party may terminate without cause on 30 days’ written notice, or for an uncured material breach after 15 days (5 days for non-payment).
New-patient packet
Employees must complete ICA’s consent and HIPAA paperwork before receiving first aid. Employers hold any shared medical information in confidence.
Your jobsite
Provide a clean, private, safe space with restroom access, running water, and waste disposal when a visit is requested.
Licensed, supervised care
ICA personnel are Colorado-licensed as required, work within scope, and are supervised by an ICA physician or medical provider.
Insurance
ICA carries professional liability of $1M per claim / $3M aggregate plus general, auto, and workers’ comp coverage. Employers maintain general liability for the service location.
